    1. Site Main Activities & High-Risk Site Works:
    • MGT- Formwork install on Level 3 pour 2.
    • Rise High- General Lifts and Subcontractor Deliveries.
    • SAS Steelfixxers- continuing with Bottom mat on Level 3 Pour 1
    • CNT-. Install/ Rough in services, on Level 3 Pour 1
    • RFS- Install/ Rough in services, on Level 3 Pour 1
    • Mosaic Plumbing- Install/ Rough in services on Level 3 pour 1.
    • APS-continuing, with placing PT ducting and pulling cable on Level 3 Pour 1.
    • Jamdev- Concrete pour Level 3 Pour 1 and internal stairs.

    2. Current Exclusion Zones:
    • Exclusion zones to be in place, to delineate workers.
    • Perimeter Scaffold
    • Current Traffic Permits in place.
    • Leading edge and signage on live decks
    • Traffic Control.
    3. Current Permits:
    • Hot Works – Mosaic, SAS, APS, MGT and RFS
    4. High Risk Plant On-site:
    • Tower Crane.
    • Delivery Trucks.
    • Concrete Pump
    • Concrete Trucks
    5. HSE issues / Other Business:
    • Exercise caution when walking across Lissner Street, to access site.
    • All Scaffold on site cannot be altered. Newnham Scaffold are the only trade allowed to alter the scaffold; alterations need to be requested by all other trades on site.
    • New Code of Practice released – Managing respirable crystalline silica (RCS) exposure in construction and manufacturing of construction elements 2022. All workers to comply, regarding new changes to managing this on site. Dust Suppression to be in place, to control dust on site.
    • Trucks to be using TGS for trucks routes, inbound and out bound.
    • No Parking in Woolworths loading bay. Workers will be accountable; their cars are towed away.
    • Trucks to turn right on Jephson street, from Lissner Street and then continue to Milton Road. DO NOT TRAVEL ON SYLVAN ROAD.
    • NO alcohol is to be consumed on site, even after work. If teams are looking to share a drink after work, then please ensure that it occurs off site.
    • There is to be no alcohol stored on site either.
    • This applies to Mosaic Staff and Subcontractors alike.
    • Toilet facilities are currently active on site.
    • Backing Props are not to be removed and if need to, then you will need to communicate with Formwork Supervisor on site.
    • Rain days: Mosaic are responsible for de watering access and egress and work areas to be dewatered by Subcontractors on site.
    • Lighting on site: Mosaic will supply access and egress areas and subcontractors will need to supply task lighting in areas of their works.
    • Temporary Electrical Boards on site are not to be tampered with on site. If the Temporary board is not working, please communicate with the Mosaic Site Management and we will communicate with the Electrical Contractor on Site.



    Site Rules for the Prescott
    1. Hard hats, protective footwear, high visibility vests/shirts must be worn on site (except inside amenities and project office areas).

    2. All construction workers shall comply with all reasonable directions of site management.
    3. Only trained, competent and authorized persons are to operate mobile plant. Operators must hold the relevant High-Risk Work License or maintain evidence of competency.
    4. Alcohol or drugs are not permitted on site at any time. Random drug and alcohol testing is conducted. Anyone suspected of being under the influence of alcohol or illegal drugs may be tested per Mosaic’s policy and procedures.
    5. No offensive language is to be used on site.
    6. Violence, fighting or instigating a fight, threatening fellow workers, abuse, or destruction of personal or company property or the display of offensive material on site is unacceptable and will result in removal from the site.
    7. Task specific Personal Protective Equipment (PPE) must be worn as specified in SWMS, Safety Data Sheets (SDS) or site signage.
    8. Glass containers are not permitted on site except in lunchrooms.
    9. All accidents, incidents and near misses must be reported immediately to the site supervisor.
    10. All injuries or persons requiring first aid treatment are to report to the person in charge of First Aid facilities. All details of the incident and treatment are to be recorded in Hammertech Incident & Injury module.
    11. Work areas must be kept clean and free of rubbish and other potential hazards, including those that may pose a fire risk, must be cleaned up promptly. All protruding nails shall be removed. Each worker is to clean their work area progressively; all waste must be removed before leaving site or moving to a new work location. Do not contaminate recyclables with general waste or other incompatible materials.
    12. Permits to work are required for any of the following activities on site:
    a. Hot work (something that could cause a fire / ignite combustible material)
    b. Work at heights
    c. Entering a confined space
    d. Creating a penetration in a surface (> 50 mm)
    e. Demolition work
    f. Isolation of electrical or gas service
    g. Excavation work (including bulk excavation and working in an excavation)
    13. Mosaic Construction have provided lighting for access. Adequate Task Lighting must be provided by subcontractors and must not produce unnecessary or unsafe glare.
    14. Dropping or throwing materials or equipment from a height is prohibited except where suitable means have been installed to catch, control, and hold such items.
    15. Formwork shall be stripped in a safe manner. DROP STRIPPING IS PROHIBITED.
    a. Formwork or other props or bracing equipment is controlled by authorized contractors to Mosaic Construction and is not to be modified and/or interfered with, any modifications must be authorized by Mosaic Construction and undertaken by relevant contractors.

    16. Barriers, fencing and/or overhead protection are used to prevent injury to other workers or public from falling objects.
    17. Scaffolding components, handrails or fencing shall not be removed unless approved by Site Management and must be replaced immediately on completion of work in the area.
    a. Scaffolding equipment is under direct control of authorized contractors to Mosaic Construction and is not to be modified and/or interfered with, any modifications needed are to be authorized by Mosaic Construction and undertaken by relevant contractors only.

    18. All ladders must be of industrial use/type and load rated to at least 120kg and be maintained in sound condition. Extension Ladders used for access must always be securely tied (top and bottom). Step ladders or fold-out ladders must have locking braces secured in place prior to their use.
    19. All mobile plant and vehicles brought to site must be ‘inducted’ i.e., inspected, and authorised by Mosaic Constructions prior to commencing use. Plant shall be operated at a safe speed and in a safe manner, in accordance with documented safe work procedures. Every owner (or hirer) of plant must ensure that the plant is registered with the state WHS Regulator and when required, maintenance records provided and that operators are appropriately licenced, qualified, and competent.
    a. Plant on site including mobile plant is to be used only in accordance with manufacturer specifications,
    b. Only persons authorised by Mosaic Construction can operate mobile plant on site,
    c. All mobile plant left unattended and/or at end of shifts will require keys removed and where possible locked,
    d. All mobile plant is to be parked in areas designated by Mosaic Construction

    20. Speed limit of 10 km/hr must be observed by all drivers of vehicles.
    21. Misuse of site safety equipment, e.g., fire extinguishers, first aid supplies, or emergency evacuation horn will lead to dismissal from site.
    22. All electrical equipment is to be tested and tagged per AS/NZS 3012 (i.e., each three months) and maintained in a safe condition. Equipment that does not have a current tag will be removed from service and placed in a ‘quarantine’ area until testing is undertaken. A register of electrical equipment must be provided to Site Management as noted in our company procedures.
    a. At the end of each shift, all electrical equipment is to be unplugged and securely stored

    23. All electrical tools and appliances must be connected to a circuit protected by a Residual Current Device (RCD), integrated into the temporary distribution boards or a portable multiple outlets with RCD protection (e.g., orange box outlet).
    Each RCD must be trip tested (button pushed) when connected to electrical equipment. Portable RCDs are to be tested and tagged each three months and the operator is to trip test it daily. Deliberate tampering with any RCD will result in dismissal from the site.

    24. An SDS for all hazardous chemicals must be provided to site management personnel, and the hazardous chemical(s) are to be recorded in the Hazardous Chemical Register, prior to the use of a hazardous chemical(s) on this site. Al relevant personnel must refer to the relevant Safety Data Sheet (SDS) when storing, using, or handling hazardous chemicals.
    25. All chemical or substance spills must be cleaned up promptly and reported to site management as an environmental incident. All efforts should be made to isolate spills from stormwater drains, gutters, or other waterways.
    26. Do not pour or allow any substances or chemical, except clean water, to flow into any stormwater drain, gutter or waterway. Paint wash-water and other waste liquids must be removed from site.
    27. Children and Pets are strictly forbidden to enter the construction site under any circumstances.
    28. Access Ways - No person is to block any access way without prior consultation with Site Management
    29. Access to site is restricted to authorised persons only. All site personnel must attend a Site Specific Induction prior to commencing work on site and must sign on daily onto the Site Attendance Register and Hammertech before commencing work. Site personnel must also sign out of the Site Attendance Register and Hammertech at the end of each working day.
    30. All visitors must report to the site office immediately upon arrival and be inducted and/or escorted by an inducted person before permitted entry to the site. All visitors must also complete the Visitors Register.
    31. High Risk Construction work must not commence unless a Safe Work Method Statement (SWMS) has been developed and then submitted to Mosaic Construction for review. Workers must comply with the control measures outlined in their SWMS.
    32. Toolbox Talks, Health and Safety Representative meetings, Health and Safety Committee meetings, or other consultation arrangements are to take place.
    33. The use of personal music players with headphones (e.g., iPods. MP3 players or similar devices) are not permitted in construction areas on site at any time. These devices may be used within the site compound areas and in site sheds/lunchrooms if their use does not affect others. If at any time there is a complaint about the sound levels or the type of music or video being played, it must cease.
    34. Know your Emergency and Evacuation Procedures; refer to the evacuation plan and emergency procedures displayed on site.
    35. Unsafe practices, acts or conditions must be reported to Site Management immediately, without any delay.
    36. SMOKING or the use of electronic cigarettes (E-cigarettes / vapes) are not permitted on site.
    37. If in doubt, stop and ask. Persons who breach these rules may be dismissed from site.
